1993 Accord EX Idle Surge When Cold

Hi,
I have a 1993 Accord EX and when I start it up cold the idle surges up to about 2500 and down to approx 1000 for about less than 5 minutes, once it gets warm it goes away and has a steady idle, Also once its warm and I put in in gear and have my foot on the brake and keep the steering wheel turned either left or right, the steering wheel vibrates quite a bit.
any ideas what could be wrong?

The first problem is the IACV and this other valve on the intake that opens when the car is cold(I forget the name of it though)
