I have an '01 Accord V6 that was throwing EGR codes about 12 years ago when it was still under extended warranty (~98k miles, so barely squeezed it in my 100k mile warranty). I read the TSB the dealer showed me and it discussed removing the intake manifold, cleaning, drilling, and installing a flow tube. The service manager said the TSB was performed.

I'm now getting a P1491, car has 250k miles on it now. Should I start troubleshooting the EGR valve itself with my multimeter etc.? or should I go straight for pulling of this intake? I'm assuming the dealer really did the TSB as directed.

Just a hunch that even though the update helps, after that time and mileage the valve might need a cleaning. Take it off and see what the carbon situation looks like. Could be causing it to stick.
