Defective control arm.
Concur with you.
The part is defective, and it is manufacturing defect in the casting process.
The casting has inherent 'sand holes' which should have been rejected by QC before machining of said castings into parts.
All castings in the similar 'pour' or batch of casts may face similar QC problem, some of which leading to eventual fatigue failure mode.
This type of fatigue failure (in maintenance speak) is not unusual in the business of casting foundries.
In a way, it is NOT a machining fault.
