I have a 2007 I-4 Accord. Whenever I remove the oil fill cap, there is a 1.5" o-ring that remains behind on top of the oil fill hole on the valve cover. Before re-installing the cap, I insert the o-ring back into groove on the cap. However, the o-ring separates again the next time I remove the cap.

I can see where a careless mechanic could easily misplace this o-ring, which might cause an oil leak.

Did Honda really design a two-piece oil filler cap or do I have a defective cap?

No, that's how they all are. I changed oil on a few Honda where that happened. I usually stick the o-ring back in and lubricated the surface so it slides instead of sticks.
