What does this error mean and how do I fix it? It stopped working for me. Picture attached.

Same message when phone trying to connect. Unpaired and repaired with same results.
 
I have had a few glitches over the Apple CarPlay too. I’m not sure if it’s the same for the 2021, but for me, it usually worked when I disconnect the charging cord and reconnected it to the phone. Other times I had to restart the car and the problem went away. So I’m guessing restarting the car when you can since it’s wireless for you guys?

Yeah I use the wireless car play. Even with a car restart nothing. I am thinking of doing a factory reset see if that solves it.

The message itself is a Android error message. The head unit is essentially an Android device. This particular message means that the app called "SmartPhoneConnectUtils" has crashed, so Android had to stop it. You can probably clear it by turning the car off, and on again. That should re-start Android, but won't do anything about what caused the crash.

There have been some similar issues, and much confusion, in the Hybrid over a similar message. There, the app that crashed was the one that displays how power is flowing between the battery, engine, and wheels. The confusing part is that the app is called PowerFlow, so the message is "Unfortunately, PowerFlow has stopped."
 
Unfortunately, restarting the car may not work because that doesn't really reboot the head unit. Try this:

To restart Head Unit:
1. Step on Brake
2. Start engine, then immediately shift to reverse by pressing Reverse button. This will turn on the reverse camera.
3. After 2 to 4 sec, shift to neutral (press Neutral button). At this point, the head unit will restart. The delayed availability of "OK" button (greyed out) on the disclaimer screen indicates that the head unit is indeed restarting. If The "OK" on the Head unit is immediately available after shifting to Neutral, the process did not work, repeat the process. Try shifting to Neutral faster.
4. Recheck the "Up time" in settings, and it should only be a few seconds at that point. Head unit has been successfully restarted

The timing of step 3 can be tricky. It can take a few tries to get it to work.

And in my experience with "Power Flow" (it's happened twice) and "Tunner" (not to me, but it affects the FM, and we seemed to think it was a mispelling unless they are brewing beer in the car), rebooting wasn't necessary. The app has to initialize, not the Android system. But, as always, your mileage may vary.

I've also never hear of that procedure working in a hybrid. The tricky part is "start engine." It has a mind of its own.

I have not seen the error message posted by the OP, but in my 2020 EX-L, CarPlay was working perfectly in the iOS14.8 era. The climate control would stop the fan and the radio would mute, and whatever mapping app I was using understood me and and I was on my way to navigating to my destination. But since upgrading through the various iOS 15.x revisions, CarPlay has been very unreliable. Now, when I want to issue a verbal command, CarPlay sits there for a few seconds, and then I usually hear "...Pardon?" I did all the things that are recommended, such as make the car forget the phone, and make the phone forget the car, uninstall/reinstall the usual mapping apps (google maps, waze), reboot the phone, and so on. ...Very disappointed. If I set up navigation manually (dangerous if I am doing it rather than someone else), the navigation off the phone works, but the Honda infotainment system will not let the mapping app talk. So I need to keep an eye on the display to see if the navigation app is trying to tell me to turn soon. Also a bit dangerous... However, the built in Honda voice works fine for choosing music on my thumb drive i keep in the car.
 
Update to my post #11...

Honda contacted me (from an email I sent them) and said that in the case where an existing iPhone that is linked to the car is updated to a newer iOS version, a "Factory Reset" should be performed on the Honda Infotainment system. Understandably, I was not happy to hear that as a solution to the CarPlay reliability problem. But, geek that I am, I took notebook paper out to the car and wrote down all the settings in the infotainment system, whether or not I changed them. I then went back in the house, opened the full owner's manual pdf file to the section the discusses the settings, reduced my changes down to those that do not appear as default values. I then did the Factory Reset, which took a minute or two to do. When it finished, I updated my settings to what I had before, and then I linked my phone to the car through Bluetooth, and then plugged in the phone to the car over a lightning cable (my model year is pre-midcycle refresh, no wireless for me...). A moment later, the "Smartphone Connection" on-screen button changed to "CarPlay" as I would expect. I pushed the newly renamed CarPlay button, chose Google Maps in CarPlay, clicked on-screen to the Google Maps search screen, tapped the little Microphone button in the upper right, the screen changed to the "Google is listening" screen, I verbally gave an address, and.... oh my gosh! CarPlay and Google Maps understood me!! The little box that contains the Google Maps "Start button" appeared over its map, I pressed it, and Google Maps started talking to me through the car speakers in a proper navigation session like it should, telling me when to turn or exit.

I really wish there was a "full strength" forget my phone (and forget the last phone's iOS version) button. ...or even a dedicated reboot button. ...lol, if Microsoft can trust users with the ability to reboot Windows whenever they want, why can't Honda? I don't think the average consumer would tolerate having to factory reset the car every time Apple updates iOS (or possibly even, when one of the gps providers updates their app)...

I counted 17 individual settings that I changed from the various default values, which includes the individual audio tone controls and fader for the audio system.

To the OP of this thread, try a Factory Reset to possibly help fix your problem.

I did end up doing a factory reset shortly after back then. Never seen it since.

Wow! Thank you! (I know is an older post but it helped me!) Tried above steps, only took me 3 times; and it fixed the error code. It’s back to connecting wirelessly! THANK YOU!!!!

This absolutely worked for this error.
I only had this issue once I started using android car plat. Never had a problem with Apple car play. Not sure if that makes a difference but thanks!

Ps. As stated if it doesn't work the first time, try shifting into neutral faster (immediately after shifting into reverse worked for me.)
