The VTC actuator on out 2017 was replaced under warranty but now several thousand miles later (and out of warranty) the start up rattle is back. Is this something that has to be fixed or is that rattle at start up the only issue? Obviously I'll get it fixed if it's going to cause damage to the engine but I can live with the rattle. Car has 97k miles now.

Which engine do you have? K24 or J35?
K24W's in the 9G are known for failing timing chain tensioners, which can cause a rattle at startup. Typically its a non issue if its only for a second or two at cold startup, but if its EVERY time you start the car, then its necessary to replace ASAP in my opinion. Not sure if the J35 can have rattle issues at startup.

No VTC actuator on a 3.5 V6. But a hydraulic timing belt tensioner can begin to make a rattle (machine gun noise) if the fluid has leaked out. This needs immediate replacement. VTC actuator with chain is more forgiving but would not ignore for to long.

K24, it's only on cold start up.
My '14 Accord CVT will do a 1 to 2 second rattle if the car sits for more than 48hrs. I bought the A02 (new version) tensioner, but the rattle hasn't gotten worse.

One thing that is noteworthy, is I added 1/2 bottle of the Marvel Synthetic oil additive, the rattle has been quieter & doesn't happen as often.

Been searching on whether the VTC Actuator affects the 2.0L(2014 Acura ILX). Does anyone know if this is the case or is the VTC Actuator issue only on the 2.4L engines? Thanks in advance!
No VTC actuator on a 2014 2.0.
